Been a while since I posted but I believe I found the issue.
As @Cody mentioned I believe it was the thread.
Replaced bobbin winder and still had issues.
Replaced thread and we are back in business.
Weird. Good thing is it was free and less than 1/2 roll.

Glad you got it figured out. It can be super frustrating chasing ghost issues.

@miket Good to hear your back to business! What brand did you end up replacing it with?

@John I'm not sure the brand. It was another roll in the bag of stuff the PO gave me.
